Key Responsibilities
- Conduct comprehensive psychosocial and mental health assessments.
- Develop, implement, and monitor individualized treatment plans.
- Provide individual and group counseling services.
- Perform crisis assessments, triage, and appropriate interventions.
- Track client progress and adjust treatment plans as needed.
- Coordinate care and make referrals to healthcare providers and community resources.
- Maintain accurate, timely, and confidential clinical documentation.
- Collaborate with interdisciplinary teams to provide integrated healthcare services.
- Support clients' overall mental health and wellness through ongoing care and education.
Qualifications
- Master's degree in Social Work (MSW) from an accredited program.
- Active New York Licensed Clinical Social Worker (LCSW) license.
- 1–2 years of experience providing clinical services in the mental health field.
- Bilingual (English/Spanish) is a plus.
- Hybrid work opportunity.
