Who We Are:
Our mission at Venture Forthe is to help people live safely and independently in the homes and communities they love.
Our Core Values are: Empathy First; Connecting People, Bridging Gaps; Be the Expert – Lead the Way; Reach Beyond Boundaries; Adapt and Thrive; Integrity in Every Action.

- Develop comprehensive Initial Service Plans (ISPs) for participants entering the NHTD and TBI Waiver Programs.
- Manage a caseload of approximately 25 participants throughout the Rochester region.
- Build collaborative relationships with participants, families, hospitals, rehabilitation facilities, providers, and community organizations.
- Conduct assessments and gather information to create person-centered, strengths-based service plans.
- Coordinate successful transitions from Initial Service Coordination to Long-Term Service Coordination.
- Serve as a subject matter expert and resource for the Service Coordination team.
- Assist leadership with mentoring, training, quality assurance, and process improvement initiatives.
- Partner with leadership to solve complex participant situations and improve service delivery.
Why You'll Love this Role:
- Collaborative leadership team that values your expertise
- Hybrid flexibility between community visits and office work
- Opportunities to mentor and develop other Service Coordinators
- Professional growth within a leading home and community-based services organization
- The opportunity to make a meaningful impact every day
Ready to Make a Difference?
If you're looking for more than just another case management position—if you want a role where your expertise directly changes lives—we'd love to hear from you.
