SKIP builds long lasting relationships with families, connecting children/youth with supports they need like nursing/home care, insurance, DME/supplies, food, transportation, housing, educational advocacy, respite, accessibility, palliative care, and other family supports. Through Care Management, intensive advocacy and hands-on problem solving, SKIP of New York Care Managers achieve remarkable outcomes, shining a light in the lives of others, ensuring the children they work with can reach their full potential, living lives of meaning and value to them.

Supervisors in the Health Home Care Management program are responsible for management and support of a designated team of care management and care navigator staff, including general oversight of assigned cases. Participate in department meetings, assisting with various policy and process development and implementation. Demonstrate time management, prioritization and organizational skills. Supervisors are responsible for team management, compliance, billing, client meeting requirements, extensive documentation requirements, quality assurance and family satisfaction follow-up. Supervisors must review and approve plans of care, CANS and other comprehensive assessments, and review site visit details, providing guidance to care managers in their support of families. Attend meetings in the community. Hold regular supervisory meetings with care managers, completing extensive case reviews, ensuring each case has ongoing comprehensive case notes to support care manager work efforts. Demonstrate command of regulatory requirements, ensure all billing requirements are met and charts kept in pristine audit ready condition. Monitor training requirements and help provide training to staff. Review/approve staff time sheets, expenses, time off requests, and complete performance appraisals. Process and approve outgoing mail. Participate in incident reporting and fair hearing follow-up as needed. Troubleshoot issues and service denials. Ensure families are well supported. Act as a liaison across families, schools, counties, Health Homes, SSA, other providers and community based groups. Assist with overall leadership and supervision within the care management program.

The qualified individual must have a Master’s degree in a health, human service or related field with one year of relevant experience; a Bachelor’s degree in a health, human service or related field with two years of relevant experience; or License as a Registered Nurse with two years of relevant experience. Demonstrated leadership/supervisory skills, experience supporting individuals with complex medical conditions and/or developmental disabilities, and prior Care Management and Early Intervention experience preferred.

Team Management-Supervision

  • Manage multiple priorities whiles supervising a group of up to 5 or more staff who may include Assistant Supervisors Care Managers and Care Navigators, supporting families according to program policy and caseload structures
  • Demonstrate strong written and verbal communication skills
  • Review Plans of Service, Reassessments and other programmatic documentation
  • Actively assist with monthly team meetings
  • Hold biweekly 1:1 supervision meetings with advisees, offering guidance and training, increasing frequency of supervision and support as necessary
  • Complete record reviews, maintaining knowledge of all caseloads of team staff
  • Schedule and conduct joint visits to client homes with new Care Managers as needed
  • Accompany team members to internal/external meetings or site visits
  • Discuss trends in caseload and care management practices/issues. Establish time frames and set deadlines for team requirements to be completed as needed
  • Facilitate case transfers and assist with client transitions
  • Support Care Managers and families with CPSE/CSE/504 plan meetings and reviews
  • Assume care manager/supervisory role if a caseload should need one and as back up for team
  • Work collaboratively with and assist Supervisors, QA, Training and Management staff as requested
  • Complete and assist with employee performance evaluations in a timely manner.

Compliance

  • Demonstrate command of regulatory requirements through timely and accurate completion of all required paperwork and programmatic contacts
  • Ensure ongoing case record and program compliance according to established policies and time frames for completion of essential case documents
  • Review physical and electronic records for active cases
  • Review billing and case notes weekly for team members and others in the department, as needed
  • Complete audits of case files and ensure care managers follow-up to address ongoing audit requirements and follow-up as requested by QA
  • Update and review tracker items in order to ensure compliance
  • Review case closures and determine cause of closure. Closure procedures must be followed and transitions must be tracked, with notifications to the Director of Programs, other Supervisors, QA and Billing Department as appropriate.

Complete program/status changes, dispo requests and update client information as needed

  • Assist with monitoring staff training requirements and ensure staff are scheduled and participate in trainings as necessary to meet program standards.
  • Review and process Child and Adolescent Needs and Strengths (CANS) assessments in UAS, including requests of additional documentation, acting as Supervisory sign-off.
  • Submission of DSS-639/5144 disability determination packets to NYS.

Physical Requirements

-Go to client homes or community locations where there may be animals present

- Climb stairs and take elevators. Use public transportation. Travel to distant counties across regions

-Lift potentially heavy client charts

-Attend IEP meetings, tours of schools/programs, attend fair/impartial hearings

-Conduct site visits and monthly meetings in various areas of New York State

REQUIREMENTS:

-The qualified individual must have a Master’s degree in a health, human service or related field with one year of relevant experience; a Bachelor’s degree in a health, human service or related field with two years of relevant experience; or License as a Registered Nurse with two years of relevant experience. Demonstrated leadership/supervisory skills, experience supporting individuals with complex medical conditions and/or developmental disabilities, and prior care management experience preferred. Excellent organization, time management and communication skills are essential to the position.

-Proficiency in MSWord and Excel with ability to effectively use various forms of technology

-Bilingual a plus

-A reliable vehicle with appropriate insurance coverage required

JOB TYPE: Full-Time, Monday-Friday, Day Shift, 8 Hour shift, Hybrid (for qualified staff)

SALARY RANGE* : $32.50-33.75/hr.

*Compensation commensurate with position type, education, relevant experience & overall qualifications.
