Care Manager - Teen Transitional Living Program

Teen Transitional Living Program (TTLP)
Care Manager Level 1
- QUALIFICATIONS
- Bachelor’s degree in Human Services or related field preferred
OR
Equivalent combination of education and experience
Requirements:
- Good oral and written communication skills
- Demonstrated leadership ability
- Good knowledge of community resources for adolescents
- Demonstrated ability to establish rapport and relationships with clients.
- Good interpersonal skills
- Valid NYS or PA driver’s license and ability to drive agency vehicle
- Must be able to lift over 40 lbs.
- Computer skills adequate to job duties
- MAJOR FUNCTIONS
- Administrative:
- Participate in staff meetings and agency trainings as required.
- Complete 40 hours of trainings as required by OCFS.
- Participate in on-call rotation.
- Participate in monthly supervision with program supervisor.
- Responsible for case record documentation and daily and monthly record keeping.
- Represent the Agency and Program in a positive manner.
- Maintain daily face to face contact with clients.
- Complete daily apartment safety checks.
- Conduct monthly fire drills with clients.
- Complete spill kit and first aid kit checks monthly.
- Participate in admission and discharge planning.
- Link clients to necessary services and act as an advocate.
- Complete individual needs assessments including an independent living assessment.
- Develop a person-centered service plan with the client.
- Provide skills training in shopping, money management, menu planning, transportation, leisure skills, personal hygiene, housekeeping, use of community services and any other IL skill training need.
- Provide education and information to pregnant and parenting youth
- Ensure that residents are safe and following program rules and expectations.
- Ensure environmental safety in TTLP residences.
- Meet with clients to update and review service plan progress.
- Intervene to resolve crisis situations.
- Provide transportation as needed.
- Assist clients in setting daily/weekly activities
- Help clients move into and out of the program.
- Coordinate with other service providers and attend meetings as necessary.
- Co-facilitate weekly ILS Group, community service projects and program activities.
- Provide Transition Planning and After Care services.
