Social Worker-Harlem Center-Institute for Advanced Medicine-Aids Non Nursing-Mount Sinai Morningside-Full Time-Days
Assesses and evaluates patients/families' social, psychosocial and environmental situations to determine patient needs. Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned.
Responsibilities- Develop options and treatment plans in a timely manner
- Provide direct service to patients and families selecting treatment modality casework technique compatible with assessment and identified goals
- Ability to identify, select, access and make referrals to appropriate community resources
- Monitor, coordinate, manage activities for high risk psychosocial patients to ensure implementation of multiple services
- Advocate for patients and families by interpreting their needs to and eliciting cooperation from other disciplines and community agencies
- Identify and assist team in managing obstacles to treatment and/or discharge plans
- Provide psycho-education for patients and families
- Knowledge of child protection, elder abuse, domestic violence policies and protocols
- Participate interdisciplinary rounds, case conferences, staff meetings
- Document in the medical record in accordance with standards set by Department of Social Work and/or regulatory agencies
- May supervise social work interns
- Masters in Social Work from an accredited school of social work. Knowledge of social work services, theories, techniques, community resources and agencies.
- Related experience in the social service and/or health care field, preferably one year in a medical setting, which can include related field placement
- New York State Department of Education Licensure – Licensed Masters of Social Work (LMSW) or Licensed Clinical Social Worker (LCSW)

Compensation
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $44.3924 - $57.1118 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
