Overview:

Position:  Registered Nurse – Home Health / Homecare
Location:  Poughkeepsie, NY 12601
Start Date:  May 18, 2026
Duration:  13 weeks (Possible Extension)
Shift:  Days, 8.5‑hour (8:30 AM – 5:00 PM, 30‑min unpaid lunch)
Hours/Week:  40 guaranteed

Job Summary:
Provide skilled home health nursing care to patients across Ulster and Dutchess Counties. The RN will manage a wide range of diagnoses and procedures, requiring strong independence and adaptability in diverse home environments. Travel is required daily (50–75 miles).

Job Duties:

• Deliver home care nursing services for patients with complex medical needs.
• Perform wound care, wound vac management, and dressing changes.
• Manage PleurX drains, nephrostomy tubes, and ostomy care.
• Provide PICC line care and dressing changes.
• Conduct blood draws and venipuncture.
• Support chronic disease management (diabetes, CHF).
• Educate patients and families on care plans, medications, and self‑management.
• Document patient care accurately, preferably using HCHB application (preferred).
• Travel daily to patient homes within Ulster and Dutchess Counties.
• Ensure compliance with infection control, safety standards, and home health regulations.

• Active NY RN license
• BLS certification required
• 1–3+ years of home care experience
• At least 1 year of recent clinical experience
• Must have reliable vehicle in good condition and ability to travel
• Preferred: HCHB (HomeCare HomeBase) application experience
