Remote Locum Care Planning Provider
We are seeking an experienced and detail-oriented Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a remote locums (per diem) role. This position focuses on chart review and care coordination, helping to ensure patients receive comprehensive, evidence-based, and well-coordinated care. The role offers flexibility, allowing you to set your schedule while supporting high-quality patient outcomes.
Responsibilities:
Review patient medical records and histories to identify care gaps and opportunities for improved health outcomes.
Develop, update, and implement individualized care plans in collaboration with the clinical team.
Provide medical input on care coordination activities to ensure comprehensive and evidence-based approaches.
Identify and recommend necessary preventative screenings, immunizations, and interventions according to best practice guidelines.
Communicate effectively with patients, caregivers, and interdisciplinary team members regarding care plans and next steps.
Accurately document findings and recommendations within the EMR system.
Participate in interdisciplinary team discussions and contribute to ongoing patient care strategies.
Requirements:
Current and valid Nurse Practitioner (NP) or Physician Assistant (PA) license.
Active New York state license
