LVN / LPN,

This Licensed Vocational Nurse (LVN) position requires providing direct patient care under the supervision of an RN in a medical-surgical/telemetry unit of a tertiary care hospital. The LVN will manage their own patient assignments, performing a variety of clinical tasks and collaborating with the healthcare team. Experience within the last two years in a hospital setting is required.

Location: Rochester, New York, United States,

Responsibilities:

- Managing own patient assignments with RN support.
- Providing direct patient care, including medication administration (oral, IM, SQ, IV), wound care, and monitoring vital signs.
- Performing procedures such as Foley catheter insertion, obtaining specimens, and administering oxygen.
- Monitoring and documenting patient status, including intake and output, pain assessment, and changes in condition.
- Utilizing electronic health records (Epic).
- Collaborating with the interdisciplinary team.
- Obtaining 12-lead EKGs.
- Performing Telemetry monitoring.
- Float to other units as needed.
- Every other weekend (EOW) night shift requirement (3x12-hour shifts).

Required Skills & Certifications:

- Current LPN/LVN license in New York.
- Minimum of two years of recent hospital experience (within the last two years).
- BLS certification.
- Proficiency in EPIC electronic health record system.
- Ability to work independently and collaboratively within a team.
- Skill in performing various nursing procedures (listed in the job description).

Preferred Skills & Certifications:

- Experience in medical-surgical and telemetry units.
- Experience with ventilators, tracheostomies, and PICC lines.

Special Considerations:

- Night shift position (3x12-hour shifts).
- Every other weekend (EOW) requirement.
- Float to other units as needed.
- Patient diagnoses include COPD exacerbations, infections, failure to thrive, and social issues.
- Patient ratios: Days 1:5, Nights 1:6, Weekends 1:6.
- Tertiary care hospital setting.

Scheduling:

- 3 twelve-hour night shifts every other weekend.
- Shift assignments are pre-determined.
