The New Broadway Family Shelter is seeking a passionate and motivated Client Care Coordinator (CCC) to join our supportive team and begin a fulfilling career serving families in crisis.

This is a unique opportunity to learn and grow while working with diverse populations—including individuals, couples, families, and adolescents—within a trauma-informed, client-centered framework. You will receive hands-on training, clinical supervision by an LCSW, and experience in key areas of social work practice including assessment, counseling, group facilitation, and community collaboration.

Position Description

The Client Care Coordinator (CCC) provides trauma-informed, strengths-based counseling and care coordination to families residing at the New Broadway Family Shelter. The CCC will assess the holistic needs of individuals and families, develop and implement clinically informed interventions, and ensure connection to internal and external supports that promote stability, resilience, and long-term success.

Key Responsibilities:

  • Conduct comprehensive family bio-psychosocial assessments and screenings (mental health, trauma, substance use, child development) from a holistic, strengths-based lens to understand family and individual needs.

  • Provide short-term counseling and support to individuals and families using a variety of evidence-based and trauma-informed approaches, including:

    • Cognitive Behavioral Therapy (CBT)

    • Conflict resolution strategies

    • Motivational Interviewing

    • Frustration tolerance skill-building

    • Psychoeducation on trauma, parenting, self-regulation, and behavioral health.

  • Support young mothers and families, addressing parenting needs, family dynamics, and access to appropriate services.

  • Identify children with developmental, behavioral, or learning needs and connect families to evaluations, Early Intervention (EI), Committee on Special Education (CSE), and specialty providers.

  • Coordinate services with schools, medical providers, and developmental specialists to ensure children receive comprehensive care.

  • Deliver crisis intervention services and ensure safety planning and follow-up.

  • Plan and implement individualized clinical interventions and community-based referrals aligned with the family’s goals.

  • Address barriers to stability such as:

    • Housing insecurity and the transition to permanent housing.

    • Domestic violence and safety concerns.

    • Mental health, substance use, and medical needs.

    • Parenting challenges and system navigation.

  • Conduct care coordination across disciplines, collaborating with internal shelter staff and external service providers.

  • Assess families for supportive housing eligibility, Complete 2010E application.

  • Facilitate therapeutic and psychoeducational groups tailored to the needs of adults, young mothers/fathers, Trauma education, Post Partum. etc

  • Co-facilitate staff trainings to support trauma-informed practices, clinical awareness, and interdisciplinary collaboration.

  • Maintain active engagement and regular contact with clients and collaterals to support goal achievement and meaningful participation in services.

  • Ensure all services and documentation comply with contractual and programmatic standards.

  • Guide the social service team in implementing clinically informed interventions for individuals and families, offering clinical insight and consultation to support trauma-informed, effective service delivery.

  • Ensure compliance with contract service requirements, including documentation, assessments, follow-ups, and service benchmarks.

  • Maintain regular contact with families and service providers to promote engagement, monitor progress, and respond to evolving needs.

  • Ensure that clients and their collaterals are actively participating in program services and supports as part of a coordinated care approach.

  • Participate in Supervision with the LCSW, Clinical Director for professional development and Clinical Supervision licensure hours towards LCSW, LMHC.

Essential Job Functions

The following duties are mandatory requirements of the job:

  1. Provide clinical assessments including bio-psychosocial assessments and other screenings of families on caseload (maximum of 25).
  2. Complete Family Bio Psycho social and all screeners
  3. Provide short-term counseling to both the primary client and other family members individually and in group settings.
  4. Provide group facilitation in the areas of parenting skills, healthy relationships, treatment adherence, mental health, activities of daily living, substance abuse, and other topics as needed.
  5. Assess clients’ needs with particular attention to family dynamics, mental health, and overall aspect of their wellbeing.
  6. Conduct multi-disciplinary case conferences with other members of on-site case management team.
  7. Provide referrals to and care coordination with Housing Specialists, Case Managers and medical and other outside social service providers.
  8. Submission of timely documentation.
  9. Provide crisis intervention.
  10. Safety Planning and Follow up.
  11. Attend housing meetings, staff meetings, clinical group, team, and individual supervision, staff and organizational meetings, and trainings
  12. Work closely with clients to encourage compliance with the program’s Code of Conduct.
  13. Appropriate and timely referrals on behalf of clients provides client advocacy in negotiating systems and delivery of services.
  14. Liaison with ACS
  15. Attend and/or coordinate case conferences with other providers.
  16. Other Responsibilities/duties as assigned

Additional Expectations:

  • Ability to build rapport with vulnerable populations, including young mothers/fathers, families with complex trauma histories,and children with special needs.

  • Skilled in delivering culturally responsive care, maintaining a nonjudgmental and respectful approach to diverse family structures, backgrounds, and experiences.

  • Comfortable working in a fast-paced, team-oriented shelter environment, balancing multiple priorities while maintaining clinical integrity.

  • Commitment to professional growth, including participation in supervision, training, and ongoing learning in trauma-informed care, behavioral health, Chronic mental health and family systems.

  • Strong commitment to equity, dignity, and empowerment in all client interactions.

Education and Certification

  1. MSW, LMSW, MMHC, LMHC, MFT
  2. A working knowledge/ background working with homeless families plus services rendered to the mentally ill and substance use populations is a plus.

Special Skills and Knowledge

In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses some of the following:

  1. Knowledge of DHS and entitlements for low-income/homeless individuals in NYC
