Descriptive Title:Patient Navigator

Full-time, Monday to Friday, 9:00am - 5:00pm, 37.5 hours per week.

Description of Duties:

This is a grant funded position. -Client engagement & outreach, provide appointment reminder calls and follow up when appointments are missed. -Care coordination with medical providers to support retention and medical treatment adherence. -Health education & group facilitation. -Individual substance use counseling sessions (will be based on qualifications). -Document all services (session notes, accompaniments, assistances, outreach, etc.) as required in the electronic health record. -Complete all primary care status measures (PCSM) updates, complete service encounter forms for entry into database. -Maintain hard-copy paper charts for all enrolled clients for specific program documents. -Participate in daily case conferences with health care team members. -Meet with Program Manager for clinical supervision at least twice monthly. -Condict home visits to encourage retention in substance use treatment, accompany clients to appointments (on and off-site), when needed.

Qualifications:

REQUIRED: High school diploma or equivalent required. Experience working as a patient educator, peer leader, case manager or related position working with vulnerable populations (i.e., people with HIV/AIDS, mental health, and/or substance use disorders). Computer literacy required; must be proficient with email systems, navigating electronic health record, etc. PREFERRED: CASAC (Credentialed Alcoholism and Substance Abuse Counselor) or CASAC-T strongly preferred. Bachelor's degree in human services, social work, psychology , or related field preferred. Lived experience with HIV and/or substance use recovery strongly preferred (persons with lived experience and expertise should be certified by the NYS AIDS Institute Peer Worker Certification Program).
