Position Overview The Grand Rehabilitation and Nursing Center at South Point is seeking a compassionate and skilled Social Worker to join our team. In this role, you will provide essential psycho-social support to residents and their families, ensuring the highest quality of life for our residents through personalized care and counseling. Key Responsibilities Resident & Family Support * Conduct interviews with residents and their families to assess psycho-social needs. * Provide individualized psycho-social assessments, counseling, and guidance to residents and their families. * Offer grief and loss counseling to residents and families experiencing difficult transitions. Care Plan Development * Develop personalized care plans tailored to the individual needs of residents, in collaboration with the healthcare team. * Assist residents in navigating and applying for appropriate social services and community resources. Crisis Intervention & Support * Provide crisis intervention services as needed to address immediate concerns or challenges. * Offer supportive counseling for emotional and psychological well-being. Communication & Collaboration * Strengthen communication between residents, families, and facility staff to ensure a coordinated approach to care. * Collaborate with other healthcare professionals to ensure holistic care for each resident. Required Qualifications * Bachelor’s Degree in Social Work or a related field (Master’s preferred). * Previous experience in a healthcare or rehabilitation setting is preferred. * Strong interpersonal and communication skills, with the ability to build rapport with residents, families, and colleagues. * A compassionate approach to care and a commitment to enhancing the quality of life for our residents. * Ability to maintain confidentiality and handle sensitive information appropriately.
