RN Health Care Surveyor
Responsible for conducting re-licensure surveys, focused surveys, complaint investigations, and associated tasks for Licensed Home Care Services Agencies (LHCSAs) The Home Care Agency Administrator is responsible for overseeing the operations of a Licensed Home Care Services Agency (LHCSA) in compliance with state regulations and standards set forth by the New York State Department of Health (DOH). This role involves managing staff, ensuring quality care for clients, and maintaining adherence to all applicable laws and guidelines. Responsibilities:
- Conducts on-site licensure surveys of LHCSAs to ensure regulatory compliance.
- Conduct on-site complaint investigations/focused surveys of LHCSAs to address complaints effectively and ensure regulatory compliance.
- Conducts interviews, home visits, and reviews of patient and personnel records, Governing Authority and quality assurance meeting minutes, policy and procedure manual, operating license, complaint log, admission packet, on-call procedure, to assess the various aspects involved in the provision of home care services as per statutory and regulatory standards.
- Writes Statements of Deficiencies (SODs) if applicable, of the surveyed agencies using the DOH’s ASPEN computer program
- Reviews site Assessment Report and discusses questions/concerns with supervisors.
- Prepares for the survey, required travel, materials, and transportation options.
- Maintains flexibility regarding workspace and environment while in the field.
- Meets co-reviewer(s) at the appointed location and time.
- Continually works to create an environment of teamwork toward common goals, mutual support and respect
- Tracks all aspects of the work process, as required by the Director, LHCSA
- Takes part in special projects and/or additional ongoing responsibilities as directed by the Director, LHCSA
- Agrees to be available for any legal proceedings that result from enforcement actions as a result of your work on this contract.
- Other tasks assigned by your manager.
- Registered Professional Nursing (RN) licensed and registered in New York State.
- Associate or bachelor’s degree in nursing.
- Two years of experience in nursing preferred.
- Knowledge of state regulations governing home care services is essential.
- Strong leadership skills with excellent communication abilities.
- Experience in home care as a RN licensed in NY.
- Demonstrates logical reasoning & critical thinking skills.
- Detail oriented, highly organized work skills
- Demonstrates a mature, professional, respectful, demeanor and style.
- Excellent verbal and written communication skills.
- Computer skills including proficiency in Microsoft Word and Microsoft Excel.
- Quality improvement orientation.
- Flexibility to travel at times for several hours.
- With satisfactory evaluation, committed to remain in position for at least one year.
