Physical Therapist (PT)
- Planning, provision and coordination of Physical Therapy to Patients of the Agency.
- Evaluating the Patient’s level of physical functioning and to work with the Patient/Family to provide training and/or assistive devices to eliminate or reduce areas of deficit, usually associated with gross motor disorders and functioning.
- Work with other professional staff to establish rehabilitation and maintenance goals and activities.
- Evaluates the Physical Therapy needs of the Patients and provides Physical Therapy Services for Patients upon receipt of an Authorized Medical Practitioner’s Order for evaluation and treatment of a physical disability, and submits written initial evaluation for inclusion in the Patient’s record.
- Performs Clinical tests to discover functional limitations and the potential of restoration and, in accordance with Physician’s orders, develops a Plan of Care with restorative and maintenance goals to meet identified needs.
- Instructs Patients, Families, Caregivers, and other health team personnel in the use of assistive modalities and appropriate Physical Therapy procedures.
- Observes, records and reports to the Authorized Medical Practitioner and Agency Personnel, the Patient’s reaction to treatment, and any changes in the Patient’s condition.
- Works with the health team to shorten the Patient’s recovery from illness and incorporate the management of residual disability.
- Communicates with the Nurse at least every two (2) weeks to discuss the Patient’s progress. Telephones the Agency immediately with any information that will influence or change the provision of services including any unusual situations observed during the course of service.
- Re-evaluates patient as needed to detect progress attained and completes a written progress report for the Patient’s record containing Patient’s progress, continues treatment plan, goals, frequency of visits, and outcomes of care provided.
- Identifies problems with the operation and safety of equipment laces in the Patient’s home and insures follow-up by the appropriate vendor and/or notifies the Agency to follow-up.
- Submits Discharge summaries at the completion of services that includes outcome of care provided, equipment ordered and any recommendation for follow-up care.
- Has graduated from a Physical Therapy curriculum approved by the American Physical Therapy Association or
- the Council on Medical Education of the American Medical Association.
- A minimum of one (1) year of experience as a Physical Therapist in an Acute Care setting, two (2) years preferred, Home Care Experience desirable.
- Demonstration of considerable knowledge in the current standards, procedures and techniques of Physical Therapy.
- Outstanding assessment and technical skills.
