POSITION: Clinical Field Supervisor, Assisted Living

REPORTS TO: Director of Patient Services

POSITION SUMMARY:

The Clinical Field Supervisor, Assisted Living is responsible for overseeing the delivery of nursing care in the assisted living communities we service. This role ensures compliance with clinical standards and regulatory requirements while coordinating patient care and coordinating, and supervising nursing staff. The Clinical Field Supervisor, Assisted Living plays a crucial role in maintaining high-quality patient outcomes and fostering a collaborative team environment.

QUALIFICATIONS:

  • NYS licensed and currently registered as a Registered Professional Nurse.
  • Graduate of an approved school of nursing and holds an applicable diploma.
  • Two years’ experience in the provision of nursing care in an acute care facility.
    • Two years of satisfactory nursing experience in a home care agency desired.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage a caseload effectively.
  • Proficient in electronic medical records (EMR) and other relevant software.

EQUIPMENT OPERATION:

Telephone (mobile and landline), computer (PC, Laptop, and Tablet), motor vehicle, copy machine, calculator, durable medical equipment and nursing equipment.

ESSENTIAL DUTIES:

  • Analyze and assess staffing needs and patterns for the provision of services.
  • Work with Human Resources to source, interview, and hire qualified clinicians.
  • Supervise, mentor, and continually evaluate a team of clinicians (RNs and LPNs), fostering a collaborative and supportive environment.
  • Assure coverage for any gaps in staffing.
  • Conduct regular field visits to assess adherence to established protocols and standards.
  • Collaborate with the assisted living personnel, physicians, therapists, and other healthcare professionals to assure adequate provision of services to the patients.
  • Collaborate with back-office management to assure the appropriate provision of services.
  • Ensure compliance with local, state, and federal regulations, as well as company policies and procedures.
  • Provide training and education to nursing staff on best practices, clinical skills, and patient care protocols, in collaboration with the Clinical Education Department.
  • Monitor and evaluate the quality of care provided to patients, addressing any issues or concerns promptly.
  • Maintain accurate and timely documentation in accordance with regulatory and organizational standards.
  • Serve as a point of contact for clinicians, patients, and the communities.
  • Assist in preparing, hiring for, and training for new facilities to be serviced.

SPECIALIZED SKILLS AND TECHNICAL COMPETENCIES:

  • Knowledge of patient assessment procedure.
  • Knowledge of patient teaching process.
  • Knowledge of pathophysiology.
  • Knowledge of medication, administration, via oral, IM and SQ.
  • Knowledge of medication interactions (drug-drug, drug-food).
  • Knowledge of the principles of therapeutic diets.
  • Knowledge of infection control practices, aseptic techniques.
  • Ability to provide skilled care as needed/ordered.
  • Ability to effectively communicate information with patients of all ages and socioeconomic levels.
  • Ability to inspire and encourage team members.
  • Knowing how to assign tasks effectively.
  • Ability to mediate disputes and find amicable solutions.
  • Ability to prioritize tasks to meet deadlines.
  • Ability to plan and oversee projects efficiently.
  • Ability to adjust strategies in response to changing circumstances.

This job requires the ability to operate a motor vehicle and travel throughout our geographical coverage area, as well as the ability to exert up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently and/or up to 10 pounds of force constantly to move objects.
