Mercy Home for Children is hiring for a Community Habilitation Manager for our new residence called Kay's Place, located in Gowanus section of Brooklyn. This new position will be under the direction of the Coordinator of Support Program, this position will provide oversight and support to individuals residing in the (10) OPWDD Individualized Supportive Housing (ISH) apartments, as well as supervise Community Habilitation Specialist. The Community Habilitation Manager ensures services are delivered in accordance with OPWDD regulations, agency policies, and each individual’s approved Life Plan (LP). This role involves direct (24 hours) on-call availability, crisis response, staff guidance, and ensuring the safety and quality of care for individuals receiving community-based support.

Requirements:

  • Supervise Community Habilitation Specialist delivering community habilitation services in home and community settings to individuals with developmental disabilities.
  • Available 24 hrs to respond to urgent issues, incidents, or emergencies as they arise, ensuring safety and regulatory compliance.
  • Oversee staff implementation of Life Plan (LP) goals and Community Habilitation Plans, ensuring goals and valued outcomes are consistently addressed. Attending and actively participating in Life Plan meetings.
  • Maintain compliance with OPWDD, Medicaid, and agency documentation standards.
  • Support staff in addressing behavioral concerns, implementing behavioral support plans, and utilizing positive behavior strategies.
  • Ensure staff follow the NADSP Code of Ethics and agency policies.
  • Conduct periodic site visits to ISH apartments to monitor service delivery and environment quality.
  • Oversees scheduling, coverage needs, times cards, PTO approvals, and coordination of staff assignments. The manager is expected to cover staff assignments as needed.
  • Participate in case conferences, team meetings, and interdisciplinary planning as needed.
  • Maintain ongoing communication with the Coordinator of Support Programs regarding staff performance, individual progress, and operational issues.
  • Assist with staff training, coaching, and mentoring to promote professional development and quality service.
  • Conducts audits & ensure corrective actions plans are implemented when deficiencies are identified, as well as maintain compliance training of supervised employees.
  • Ensure safety protocols, health requirements, and risk management procedures are followed in all ISH apartments.

Minimum requirements:

  • Associate's Degree in human services, psychology, social work or related field is required. BA Degree is preferred.
  • Minimum 1-2 years of experience working with individuals with developmental disabilities, including supervisory experience
  • Knowledge of OPWDD regulations & best practices in community habilitation is required
  • Excellent organizational, documentation and communication skills
  • Strong computer skills, Microsoft Suite & Excel is required, knowledge of Therap is helpful
  • Be a self-starter and able to handle multiple priorities
