The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health, the Bureau of Brooklyn Neighborhood Health, the Bureau of Harlem Neighborhood Health, the Bureau of Chronic Disease Prevention, the Bureau of Health Equity Capacity Building, the Bureau of Equitable Health Systems and the Bureau of Finance, Administration and Services. The division's Deputy Commissioner serves as the Agency's Chief Equity Officer and oversees Race to Justice, the Agency's internal reform effort to help our staff learn what they can do to better address racial health gaps and improve health outcomes for all New Yorkers. The division's Deputy Commissioner also serves as the Agency's Chief Medical Officer. The Bureau of Brooklyn Neighborhood Health: Formerly known as the Brooklyn Neighborhood Health Action Centers, the Bureau of Brooklyn Neighborhood Health will continue to provide oversight and leadership in programming and planning for the Brownsville Neighborhood Health Action Center will satellite programming and planning also occurring at the Bedford and Bushwick District Health Offices. This bureau will support programming, planning and research opportunities to address racial inequities resulting in premature mortality in Central Brooklyn.


The Bureau of Brooklyn Neighborhood Health seeks a Program Manager for" Creating Healthy Schools and Community" (CHSC), a New York State Department of Health-funded initiative. This role function as a senior programmatic and operational lead embedded within the Brownsville Neighborhood Health Action Center, supporting integrated school-and community-based strategies that advance chronic disease prevention, physically activity, nutrition access and health equity.


DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:
Systems Building & Data Management:

  • Oversee data collection, monitoring, and reporting for programmatic, community engagement, and evaluation activities. Utilize Public Health Partners Connect (CRM) and other data systems to track participation, partnerships, and outcomes. Integrate CHSC data with broader Action Center reporting and wellness tracking systems, including incentive-based engagement initiatives.
  • Prepare reports, dashboards, and presentations for internal leadership, funders, and external partners.
  • Present neighborhood health data to partners to inform shared strategies and collective action.
Community & School Partnership Development:

  • Develop and maintain strong relationships with schools, community-based organizations, city and state agencies, and local leaders; Build and support coalitions to leverage resources, strengthen coordination, and increase program sustainability. Provide training and technical assistance to partners implementing CHSC aligned activities. Program Leadership & Grant Management:
  • Serve as programmatic and operational lead for CHSC implementation within the Action Center ecosystem. Manage all grant-related activities including budgeting, reporting, contracting, staffing and compliance.
  • Translate grant deliverables into actionable workplans aligned with site-wide priorities and time lines.
  • Support program sustainability and continuous improvement efforts.
Program Implementation & Coordination:

  • Plan and execute community and school-linked wellness activities, events and presentations (Some evenings and weekends required). Coordinate vendors, facilitators, and partners supporting physical activity, nutrition, and wellness initiatives. Support integrated school-community programming that reflects neighborhood needs and lived experience. Staff Supervision. Supervise and support professional development and skill-building among program staff.
PREFERRED SKILLS:

  • Experience managing complex, community-based public health programs
  • Strong background in health equity, chronic disease prevention, and community wellness
  • Demonstrated experience with grant management, budgeting, and contract oversight
  • Experience working with schools and community-based partners in underserved neighborhoods
  • Strong data analysis, reporting, and presentation skills
  • Familiarity with CRM systems, Salesforce, Tableau, and data dashboards a plus
  • Excellent written, verbal, and interpersonal communication skills
  • Strong organizational skills with the ability to manage multiple priorities
  • Proficiency in Microsoft Outlook, Word, PowerPoint, and Excel
  • Ability to engage effectively in public forums and cross-sector partnerships

Minimum Qualifications


1. For Assignment Level I (only physical, biological and environmental sciences and public health) A master's degree from an accredited college or university with a specialization in an appropriate field of physical, biological or environmental science or in public health.
To be appointed to Assignment Level II and above, candidates must have:
1. A doctorate degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and one year of full-time experience in a responsible supervisory, administrative or research capacity in the appropriate field of specialization; or
2. A master's degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and three years of responsible full-time research experience in the appropriate field of specialization; or
3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least a master's degree in an appropriate field of specialization and at least two years of experience described in "2" above. Two years as a City Research Scientist Level I can be substituted for the experience required in "1" and "2" above.
