The Patient Care Liaison functions as a Care Manager responsible for managing high-risk patients with chronic illnesses. This role promotes effective patient education, self-management support, and timely coordination of healthcare services to improve outcomes.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Supports the mission and vision of the organization and serves as a key resource to patients and providers.

RESPONSIBILITIES :

• Promote a safe, cooperative, and professional healthcare environment
• Adhere to all organizational policies and procedures
• Provide excellent customer service
• Coordinate care between providers
• Identify patients for care management
• Provide patient education and support
• Maintain communication with patients
• Assist with scheduling and services
• Develop and monitor care plans
• Document in EMR systems
• Track outcomes and reporting
• Refer patients to resources
• Attend meetings and perform duties as assigned

SALARY

• Starting Salary: $50,000 – $55,000 annually, commensurate with education, experience, and qualifications.

QUALIFICATIONS:

• Bachelor’s degree preferred OR High School Diploma with MA certification
• Experience in care management preferred
• Strong communication and organizational skills
• Knowledge of health conditions
• Proficiency in Microsoft Office and EMR systems
