✅ Qualifications

✔️ Master’s Degree in Nursing (MSN)
✔️ Active New York State Nurse Practitioner License
✔️ Board Certification (Required)
✔️ New Grads Welcome!
✔️ Proficiency in eClinicalWorks EMR
✔️ Excellent communication & independent decision-making skills

✅ Key Responsibilities

✔️ Provide hands-on, compassionate care to nursing home patients
✔️ Conduct physical assessments, develop treatment plans, and diagnose conditions
✔️ Prescribe medications and therapies based on patient needs
✔️ Collaborate with interdisciplinary teams to ensure high-quality care
✔️ Educate patients and families on disease management and prevention
✔️ Maintain accurate electronic health records in compliance with regulatory standards
✔️ Uphold best practices and facility guidelines at all times
