Required: NY State Licensed Master Social Worker (LMSW) | Minimum 1 year hospital-based experience (psychiatric or CPEP preferred)

Position Summary

"Navitas Healthcare, LLC" is seeking a dedicated Licensed Master Social Worker (LMSW) to provide professional social work services within an Adult Comprehensive Psychiatric Emergency Program (CPEP) . This role involves performing patient assessments, crisis intervention, discharge planning, and care coordination in a dynamic behavioral health environment. The LMSW will work closely with a multidisciplinary team to deliver compassionate, patient-centered mental health services.

Responsibilities

• Conduct psychosocial, crisis, and risk assessments for adult psychiatric patients.

• Develop and implement individualized care and discharge plans with the healthcare team.

• Deliver crisis intervention, counseling, and emotional support to patients and families.

• Collaborate with physicians, nurses, and community resources to ensure care continuity.

• Maintain accurate and compliant clinical documentation per hospital and state standards.

• Participate in interdisciplinary team meetings, case conferences, and treatment planning.

• Advocate for patient rights, safety, and access to community-based resources.

• Handle high-stress psychiatric situations with empathy, professionalism, and sound judgment.

• Uphold ethical practice standards, confidentiality, and patient-centered care.

• Stay informed on current best practices in social work and crisis intervention.

Qualifications

• Education: Master’s Degree in Social Work (Required)

• Experience: 1+ year of hospital-based LMSW experience (Adult CPEP or psychiatric emergency preferred)

• Licensure: Active NYSED LMSW License (Primary Source Verification Required)
