Full-time post-acute care role for an NP or PA providing primary care services in skilled nursing facilities. This position offers strong clinical support, autonomy, and uncapped earning potential in a collaborative care model.
Position Highlights

Key Responsibilities
- Perform patient assessments, H&Ps, and care planning
- Manage acute and chronic conditions
- Coordinate care with physicians and interdisciplinary teams
- Educate patients and families
- Maintain timely and accurate documentation
Compensation & Benefits
- Competitive FFS compensation with uncapped earnings
- Estimated annual compensation: $138,334–$164,001 (uncapped productivity)
