Key Responsibilities
- Perform patient assessments, H&Ps, and care planning
- Manage acute and chronic conditions
- Coordinate care with physicians and interdisciplinary teams
- Educate patients and families
- Maintain timely and accurate documentation
Compensation & Benefits

- Estimated annual compensation: $138,334–$155,000 (uncapped productivity)
