Physical Therapist Job Details

• Location: Eureka, CA 95501 (California North Coast)
• Setting: Hospital-affiliated Home Health
• Employment Type: Travel Physical Therapist Contract
• Duration: 13 weeks (Start: 9/29/2025 – End: 12/28/2025)
• Schedule: Monday–Friday, 8:00am–5:00pm (8-hour day shifts)
• Hours per Week: 40
• Estimated Weekly Salary Range: $2,350 - $2,410
• Caseload: Geriatric patients; up to 6 home health visits per day (30 productivity points standard)
• Travel Radius: Occasional maximum of 50+ miles; patient visits grouped to minimize drive time

Physical Therapist Qualifications

• Active California Physical Therapist (PT) license
• At least 1 year of recent skilled home health experience (within the last 3 years)
• Proficiency with OASIS documentation required
• BLS certification (American Heart Association preferred)
• Reliable personal transportation for patient visits

Physical Therapist Responsibilities

• Deliver comprehensive physical therapy services to geriatric home health patients
• Perform patient assessments, develop individualized care plans, and provide therapeutic interventions
• Complete OASIS documentation for start of care and ongoing patient progress
• Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
• Manage a caseload of up to 6 home health visits per day
• Travel within assigned radius in Eureka, CA, with efficient scheduling to reduce windshield time
