Home Care Marketing Liaison (Pediatric / OPWDD)
- Build and maintain referral relationships focused on pediatric and OPWDD-funded home care services
- Identify and proactively reach out to individuals and families who may qualify for OPWDD services, and provide guidance on how to begin the eligibility and enrollment process
- Conduct regular in-person outreach to care managers, Care Coordination Organizations (CCOs), community organizations, schools, therapy providers, and other referral sources
- Educate referral partners and families on agency services, eligibility requirements, and the intake process
- Serve as the primary point of contact for referral sources and support ongoing relationship management
- Coordinate closely with intake to facilitate timely starts of care
- Attend community events, resource fairs, and networking opportunities relevant to pediatric and OPWDD populations
- Represent the public relations the agency professionally and ethically in all community interactions
- Strong interpersonal and communication skills
- Comfort with in-person outreach and community-based work
- Highly organized with strong follow-through and time-management skills
- Ability to work independently and manage a defined territory
- Professional demeanor and ability to represent the agency externally
- Valid driver’s license and reliable transportation
- Experience in home care, healthcare marketing, community outreach, or care coordination
- Familiarity with OPWDD services, pediatric populations, or Medicaid-funded programs
- Existing relationships with care managers, CCOs, or community organizations
- Knowledge of referral-based healthcare systems
- Multilingual abilities or cultural familiarity with the communities served
