Role specifics and benefits: In this case management RN role, you’ll perform comprehensive patient assessments, identify barriers to care, and develop individualized plans that align medical needs with social supports, payer requirements, and community resources. You’ll coordinate with nurses, physicians, social workers, home health providers, and rehabilitation services to ensure timely referrals, appropriate resource utilization, and safe, efficient transitions from inpatient to outpatient care.
