adjunct Case Manager

We are currently seeking a highly motivated adjunct Case Manager to join our team in Hawthorne, NY.
The ideal candidate will have an Associate's degree in Healthcare, Social Work, or Psychology, or a valid New York State LPN license. The adjunct Case Manager will work Monday to Friday during the day shift, from 8 am to 4 pm, and will be compensated at a rate of $25-$30 per hour. This is an in-person position only; no remote work will be considered.

adjunct Case Manager Qualifications required

»Associate's degree in Healthcare, Social Work, or Psychology OR a valid New York State LPN license

» 1 year of practice in case management, healthcare coordination, or a related field preferred

»Excellent communication and interpersonal expertise

»Strong organizational and time management expertise

» capability to work independently and as part of a team

»Proficient with Microsoft Office and electronic medical record systems

» capability to travel to various locations as needed

adjunct Case Manager Responsibilities:

»Assist the Case Manager in managing a caseload of clients with complex medical and social needs

»Coordinate services and resources to meet the needs of each client

»Help clients navigate the healthcare system and access appropriate services

»Work collaboratively with healthcare providers, social service agencies, and community organizations to ensure continuity of care

»Ensure that clients receive timely and appropriate care, and follow up to ensure that care goals are met

»Document all interactions with clients and providers in conformity with established policies and procedures

»Participate in case conferences and meetings with other healthcare providers as needed

»Perform other tasks as assigned by the Case Manager
