Job Summary We are seeking a dedicated and client-centered Clinical Intake Coordinator to join our team. This role is responsible for overseeing various aspects of client intake, case management, and administrative tasks to ensure smooth operations and high-quality service delivery. The ideal candidate will have experience in harm reduction, client engagement, and program development, with strong organizational and leadership skills. Key Responsibilities- Supervise unit operations as needed.
- Assess all referred clients to determine appropriateness for admission.
- Manage intake and discharge data entry, ensuring accurate client assignment to caseloads.
- Network with referral sources to facilitate client placements and program success.
- Serve as the senior on-site manager on Sundays and as needed during the week.
- Provide case management for a small caseload of clients.
- Facilitate one group session to support client engagement and program goals.
- Assist in the development and implementation of program goals as part of the management team.
- Provide emergency first aid and CPR assistance when needed.
- Submit a daily report to DHS (7 days a week) in compliance with program requirements.
- Respond to hospital calls related to client referrals and care coordination.
- Welcome all new intakes, provide program orientation, and conduct facility tours.
- Develop initiatives to improve client retention and engagement.
- Support clients in their transition to the Safe Haven, ensuring a client-centered and harm reduction approach.
- Oversee and manage intake packets for new clients.
- Assist with chart audits to ensure documentation accuracy and compliance.
- Utilize SOAR training to assist eligible clients with disability benefits applications.
Perform other related duties as assigned.
Qualifications & Skills- Experience in case management, intake coordination, or program supervision within a social services setting.
- Strong understanding of harm reduction and client-centered care approaches.
- Ability to work independently while also collaborating with a multidisciplinary team.
- Excellent organizational, communication, and problem-solving skills.
- Proficiency in data entry and report submission, with attention to detail.
- CPR/First Aid certification (or willingness to obtain).

Hours Full time, 37.5 hours per week- Monday - Friday 8am - 4:30pm?
QUALIFICATIONS: MSW or related Masters degree required. Excellent communication and writing skills, and computer proficiency required. Must be well versed in assessing individuals using the DSM IV-TR diagnostic criteria. Experience working in residential treatment and with the MICA and homeless populations preferred. Supervisory experience also strongly preferred. Bilingual Spanish a plus. CPR training certification or willingness to take training class in CPR.
