POSITION SUMMARY: Perform duties that require knowledge, skills, and abilities as a licensed professional Nurse (LPN). Assists the agency to maintain quality care and DOH regulations. Able to work independently and within a Nursing and/or interdisciplinary team. Accepts supervision and guidance from the Nurse Manager and/or the Director of Nursing.
RESPONSIBILITIES:
Continuity of Care
- Under the supervision of the Nurse Manager/Director of Nursing, assists with the assessment of patients, planning of patient care, implementing care plans, and the evaluation of patient care.
- Reviews completed clinical documentation to assess consistency with individual client care plans.
- Collaborates with RN nursing and Intake staff to assure that all submitted initial nursing documents are completed properly and entered into the system in a timely manner.
- Tracks the completion of all initial visits to ensure appropriate assignment of the nurse and timely completion of the visit.
- Assists with the investigation of client incidents, complaints, as determined by the Nurse Manager/Director of Nursing.
- Reviews clinical documentation as required for all CABS HAS employees. Conducts follow-up as necessary for corrections and implementation of continuity of care.
- Identifies and supports Value Based Care concepts into all aspects of clinical documentation and performance of nurses and home care workers.
- Uploads nursing visits to client charts as needed.
- Collects data on common errors and omissions and presents at nursing meetings.
- Provides feedback to the Nurse Manager and/or the Director of Nursing on trends occurring in clinical documentation including difficulties with completing documentation, timeliness of clinical documentation and nursing visits, and any issues relating to the compliance of nursing documentation and service delivery.
- Assists actively with the remediation of any issues with the data management system; able to learn the documentation system and seek assistance from customer service to resolve any issues with the technical aspects of nursing documentation and input.
Home Care Worker Supervision
- Conducts supervisory home visits to support ongoing client care as determined by the Quality Service Supervisors during the initial assessment visit. Documents all supervisory home visits as required.
Home Care Worker Supervision continued
- Provides direct on-site observation and supervision, via home visits, of temporary Home Attendants until the employees become permanent CABS HAS staff.

- Provides off-site evaluation, via telephone, of temporary Home Attendants as supervision until the employees become permanent CABS HAS staff. Conducts any/all necessary follow-up.
- Documents all evaluation and supervision sessions as required.
- Conducts random and unannounced home visits, as assigned by the Nurse Manager/Director of Nursing, for the purpose of monitoring the attendance and or performance of home care employees.
Continuous Quality Improvement
- Participates in the Continuous Quality Improvement activities under the supervision of the Nurse Manager/Director of Nursing
- Establishes and ensures implementation and completion of any/all corrective action plans for individual client cases as necessary.
- Participates in chart reviews and client case conferences as required.
- Conducts internal chart audits as a monitoring tool to ensure the appropriate documentation by all staff per Agency procedures and all external mandates.
- Collaborates with the Quality Assurance Department to ensure the implementation of recommended improvements and post audit corrective action plans.
- Actively participates as a member of the agency Audit Team to ensure the correct submission of documents required during all in-person and remote audits[O1].
- Participates in the Nursing Quality Oversight Committee to assist in the identification of issues, corrective actions and implementation of recommendations to improve the quality of the Nursing department function.

Job Type: Full-time
Salary: $62,000.00 - $65,000.00 per year
