About the Role

• As a Physical Therapist, you’ll deliver skilled treatment plans across a continuum of care—including outpatient rehab , skilled nursing , and assisted living settings. Whether you’re a seasoned PT or a recent graduate, you’ll find the mentorship, support, and collaboration you need to grow your career and make a lasting impact.

Key Responsibilities

• Initiate and implement personalized physical therapy programs based on physician orders

• Maintain detailed evaluations, progress notes, and discharge summaries

• Collaborate with nursing and rehab staff to enhance residents' functional outcomes

• Contribute to interdisciplinary care planning meetings

• Maintain treatment documentation, track patient statistics, and request necessary supplies

• Participate in or lead staff training and education

• Provide leadership support in the absence of the Director of Physical Therapy

What We’re Looking For

• Graduate of an accredited Physical Therapy program

• Active NY State Physical Therapy license

• Strong interest in working with geriatric, subacute, and adult patient populations

• Previous experience preferred (but not required— new grads are encouraged to apply!)

• Leadership qualities and ability to direct support staff when needed
