Job Summary

We are looking for individuals to join our client’s team as Case Worker. Under supervision, you will provide social services to clients/patients, assist with discharge planning and follow-up, coordinate aftercare appointments, perform data entry, accompany patients to screenings and appointments, and support resource linkage efforts.

Job Description

  • Conduct interviews and make initial assessments and referrals.
  • Manage cases of social service recipients.
  • Make home visits to adults to assess needs and provide support.
  • Investigate and verify information obtained from clients and other sources.
  • Provide health-related and social services to clients.
  • Assist social workers with discharge planning and follow-up care coordination.

 

Skills Required

  • Minimum of 3 years’ experience in social services, case management, or related field.
  • Experience placing patients in nursing homes and sending out referrals.
  • Knowledge of social service programs and community resources.
  • Strong interpersonal and communication skills.
  • Ability to work with individuals who have difficulty expressing their needs.
  • Organized, practical, and able to work collaboratively.
  • Ability to compromise, negotiate, and work effectively with others.
  • Careport and EPIC experience preferred.
  • Candidate will be fully trained within the department.

Education/Training/Certifications

  • Bachelor’s Degree required from an accredited college or university

Additional Requirements

  • Shift Time: 9:00 AM-5:00 PM

Pay Rate

  • Up to $43.00 per hour depending on experience
