Seven Healthcare are seeking a skilled Registered Nurse Case Manager specializing in Case Management for a travel assignment in New York. As a Level 2 Case Manager, you will play a key role in planning and coordinating patient care, ensuring seamless service delivery in a home health care setting. This role requires you to work both independently and collaboratively within a supportive environment.

Key Responsibilities of the Registered Nurse Case Manager

• Develop and revise treatment and discharge plans with collaboration from patients, family, and medical team
• Complete and submit OASIS assessments and other required documentation
• Notify physicians of patient needs and changes, and manage orders
• Provide a respectful and confidential care environment
• Consult and educate patients and families on disease processes and self-care
• Oversee and ensure quality of patient care services
• Attend patient care conferences and manage required documentation
• Contribute to operational and financial objectives of the agency

Requirements of the Registered Nurse Case Manager

• RN License (NY License ONLY)
• One year of experience as a clinical nurse in a hospital; 9 months of home care experience preferred
• Certified Home Health Care experience
• Experience with OASIS and Telemetry
• BLS Certification
• Critical thinking, clinical judgment, and communication skills
• EPIC or Netsmart experience preferred
• Current NYS driver's license
• Ability to work independently and adapt to changes
