Support Long-Term Functional Health in the Yonkers Community

@Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation services to adult patients throughout Yonkers. This role focuses on helping individuals maintain mobility, manage ongoing physical limitations, and improve functional safety in their home environments.
This location offers enhanced earning potential and is well-suited for therapists looking to build a steady, community-based caseload.

Compensation & Incentives:

• $500 bonus after 50 completed visits.
• Additional earning incentives available for this location.

What You’ll Be Doing:

Functional Mobility Evaluation:

• Assess gait, balance, strength, and overall movement efficiency in the home setting.
• Identify barriers impacting safe daily activity performance.

Ongoing Rehabilitation Support:

• Develop individualized care plans focused on maintaining or improving functional ability.
• Support patients with chronic conditions, age-related decline, or post-acute recovery needs.

Home-Based Movement Training:

• Guide patients through real-life activities such as transfers, walking safety, and stair use.
• Reinforce safe movement strategies to reduce fall risk.

Progress Tracking & Adjustment:

• Monitor changes in mobility and functional status over time.
• Modify treatment plans based on patient progress and goals.

Clinical Documentation:

• Complete visit notes using an electronic EMR system.
• Maintain compliance with clinical and regulatory standards.

Work Structure & Environment:

• Flexible Scheduling: Choose your availability and manage your caseload independently.
• Community-Based Practice Model: Provide care directly in patients’ homes across Yonkers and surrounding areas.
• Operational Support: Administrative coordination and scheduling assistance provided.
• Scalable Caseload: Opportunity to increase visit volume based on preference and availability.

Qualifications:

• Active New York State Physical Therapist license.
• Experience with adult or geriatric populations preferred.
• Strong clinical reasoning and functional mobility expertise.
• Ability to work independently in a home care setting.
• Effective communication and patient engagement skills.
