Role Summary
Our client, a premier private-pay home care agency , is seeking a Nurse Supervisor – Home Care to oversee clinical services and ensure high-quality care delivery for patients receiving care in their homes.
This role serves as a primary point of contact for patients, families, and caregivers, coordinating services and ensuring care plans align with patient needs. The Nurse Supervisor will conduct patient assessments, supervise care teams, and maintain ongoing communication with clients and families to promote positive outcomes and a high level of satisfaction.
Key ResponsibilitiesConduct initial comprehensive assessments of patients and families to determine appropriate care services and support needs.
Develop and coordinate care plans with patients, family members, and caregiving teams.
Serve as the primary liaison for patients and families, addressing questions and ensuring a high level of client satisfaction.
Supervise and support professional and paraprofessional staff providing home care services.
Monitor care delivery to ensure services meet clinical standards and patient needs.
Maintain ongoing communication with clients and families to ensure quality care outcomes.
Evaluate caregiver assignments and determine appropriate staff placements based on patient needs and care requirements.
Registered Nurse (RN) license in New York State (NYS).
Bachelor of Science in Nursing (BSN) required.
Minimum of 3 years of clinical nursing experience.
Home care experience required.
At least 1 year of supervisory or leadership experience.
Strong observation, clinical assessment, and problem-solving skills.
Excellent communication and interpersonal abilities.
Demonstrated compassion, professionalism, and integrity.
Experience working in private-pay home care settings .
Prior experience coordinating multidisciplinary care teams.
