Job Title: LVN / LPN – Long-Term Care (LTC)

Location: Vestal, NY 13850

Duration: 16 Weeks

Shift: Day or Evening (5x8 Schedule)

• Day Shift: 06:30 AM – varies
• Evening Shift: 2:30 PM – 11:45 PM
• Every other weekend required

Guaranteed Hours: 40 Hours/Week

Job Summary:

We are seeking a compassionate and skilled LVN/LPN to provide quality nursing care in a long-term care and rehabilitation setting. The role involves supporting a diverse patient population, including individuals with Alzheimer’s and Dementia , while ensuring high standards of patient care and safety.

Key Responsibilities:

• Perform medication administration (med pass) and prescribed treatments
• Monitor and record patient vital signs and overall condition
• Provide care to residents in a Rehab and nursing facility
• Support patients with Alzheimer’s and Dementia
• Maintain accurate and timely patient documentation
• Collaborate with interdisciplinary healthcare teams
• Educate patients and families on care plans and treatments
• Follow infection control and safety protocols

Required Qualifications:

• LPN Diploma
• Active New York (NY) LVN/LPN License (required at submission – no restrictions)
• BLS Certification required
• Minimum 1+ year of LTC experience (flexible)

Accepted Experience:

• Skilled Nursing Facility (SNF)
• Rehabilitation
• Med-Surg or Hospital experience

Additional Requirements:

• First-time travelers accepted
• Local candidates accepted
• COVID-19 vaccination required
• Must be available to work every other weekend

Preferred Skills:

• Experience with PointClickCare EMR (not required)
