Health Educator

Job Summary:
The Health Educator is responsible for performing routine education and HIV/STI testing on the Mobile Medical Unit (MMU), and in the community, as well as assisting with enrollment of new clients into the Reproductive, Education, Advocacy and Prevention (REAP) program. Work will be performed according to community event schedule, which may include nights and weekends, based on consumer needs and program changes.
Responsibilities:
- Coordinates and participates in Rapid HIV/HCV testing and counseling within the outpatient settings and in the community.
- Collects data on HIV testing, STI screenings, linkage to care for HIV/HCV/STI positive patients identified through testing program
- Provides and coordinates partner services for HIV positive patients
- Responsible for timely and comprehensive completion of services and documentation
- Works in coordination with the Ally Care Center, SAAVE and gynecology REAP teams to develop and maintain a collaborative curriculum
- Performs outreach to Community Based Organizations and others to develop schedule of educational events in coordination with the Ally Care Center and SAAVE program existing Memorandums of Understanding.
- Responsible for identifying new locations for educational curriculum to be delivered and testing/contraceptive services to be performed.
- Develops schedule in coordination with MMU driver of testing events
- Facilitates linkage to care as identified on MMU to REAP programs specific teams for contraceptive, HIV/HCV/STI treatment and prevention, and violence prevention services.
- Responsible for completing initial intake packet, service plan, and updated re-assessment and service plan
- Provides feedback from areas to determine best locations to provide testing to the communities with at risk individuals
- Provides outreach to Community to provide Sexual Violence prevention and identify victims for direct advocacy services
- Coordinates victims of sexual violence with WMC Sexual Assault Abuse and Victim Empowerment [SAAVE] program for comprehensive follow up advocacy services
- Provides program support to Administration as needed including preparing for audits and quality improvement initiatives
- Attends weekly case conference to discuss enrolled patient’s needs and collaborate with ACC team
- Attends monthly grant related meetings to discuss data reports, schedule of events, specific patient needs, and quality improvement projects
- Effectively uses automated systems to perform work assignments
- Assists in other duties as assigned
- Access to reliable transportation for community events, required. Reimbursement will be based on travel and expense policy
Experience:
A minimum of 2 years of experience working in an HIV treatment/preventionclinic setting, required Bilingual/Spanish speaking ability preferred
Education:
A Bachelor’s degree in human services preferred with two years of health education and HIV testing Experience,required.
Licenses / Certifications:
Certification as WMC Sexual Assault Abuse and Victim Empowerment [SAAVE] Rape Crisis Advocate (pre-certification and annual training required); can be obtained upon hire Certification as Lethality Assessment Program Advocate with Westchester County Domestic Violence High Risk Team; can be obtained upon hire
