We’re hiring a Family Nurse Practitioner (FNP) to join our dynamic Primary Care team. Whether you’re seeking part-time flexibility or a full-time position, this role is ideal for an experienced FNP passionate about community health, preventive care, and delivering equitable healthcare in a welcoming outpatient setting.

What You’ll Do

• Provide compassionate primary care to a diverse patient population
• Diagnose and manage acute and chronic conditions
• Perform physical exams and wellness screenings
• Educate patients on disease prevention and healthy living
• Develop individualized care plans and prescribe treatments
• Collaborate with care teams and refer patients as needed
• Document care efficiently using EHR with support from scribes and medical assistants

What We’re Looking For

• Master’s degree in Nursing (APRN)
• Active Family Nurse Practitioner (FNP) license in New York
• DEA certification with prescriptive authority
• Spanish language skills preferred
• 1–2 years of Primary Care experience preferred
• Strong clinical, communication, and teamwork skills
