developmental disabilities, epilepsy, and brain injury through progressive, tailored
care to improve their health, wellbeing, independence, and social inclusion.
Job Purpose:
Assists person supported and manager of service with implementing their Self-Direction budgets by providing ongoing training and support to individuals to gain and maintain the skills necessary to manage their self-directed services.
Essential Functions:

Provide ongoing training and support to person supported and COS in order to gain and maintain the skills necessary to manage self-directed services.

Participation in annual and/or semi-annual Life Plan meetings per person supported preference (in person or virtually).

Maintain documentation standards in electronic health records system.
-
Direct contact for person supported and manager of service, this includes serving as a member of their circle of support (COS) as assigned per caseload.

Monitor budget activity to ensure expenses are within Personal Resource Account (PRA) and meet all OPWDD regulations.

Remain educated on all regulations and guidance associated with self-directed services (including but not limited to; quarterly Webex, ADM's and SD Guide for Providers).

Reporting any incidents or compliance concerns to appropriate personnel.

Consults with Program Managers/Supervisors to ensure consistency of services.

Performs duties assigned by Self-direction Supervisor and/or designee within scope of practice.
Support Broker
-
Follow and adhere to all regulatory guidelines as published by the New York State Office for People with Developmental Disabilities (OPWDD) and Medicaid.

Perform all duties agreed upon within each individuals support broker agreement.
-
Attends and facilitates, when applicable, all Circle of Support meetings as required by OPWDD.

Completes all Staff Action Plans in accordance with OPWDD regulations.
-
Maintain Authorization Standards for Support Brokers per 19-ADM-05R. (This includes annual 12 hours of training and recertification).

Completes all documentation as required per Service Documentation for Support Broker Services per 19-ADM-06
-
Maintain consistent weekly and monthly billing standards for all persons on caseload.

Assist with recruiting, hiring and training of self-hired staff.
Coordinator
-
Management of compliance related documents assures the timely and accurate completion of plan for individuals receiving services per OPWDD and other regulatory requirements. Including but not limited to, annual forms, staff action plan, Life Plan, budget and all other supporting documentation.

Perform duties associated with the agency's employer of record responsibilities. This includes assisting individual's with managing their staffs job assignment, wage/status, and onboarding.

Perform at minimum quarterly check in calls to ensure satisfaction with services.
Non-Essential Functions:
Other duties as assigned within scope of practice.

Participate on various EPI committees as able.
Compliance:
-
Must maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to board/vendors/donors' names, investigations, finance, operations, and employee relations matters.

Must be willing to complete specific company required trainings as mandated by our governing agency, Office for People with Developmental Disabilities (OPWDD), at time of hire and annually thereafter.

Must adhere to all departmental /company policies & procedures at all times.
Education and Experience:
-
Min. High School Diploma and 5 years' experience in the human service field OR 2-year degree in a related field and 4 years' experience in the human service field OR Bachelor's Degree in a related field and two years' experience in the human service field.

Authorized OPWDD broker strongly preferred.
Computer Skills :
-
Strong working knowledge of Microsoft programs to include Email, Word, Excel, PowerPoint, Teams.
Qualifications:

Must possess and maintain a valid NYS Driver's License-a vehicle is required for business travel

Solid verbal and written communication skills and ability to communicate with all persons supported and a diverse employee population of various levels within the organization.

Demonstrated ability to use poise, tact, maturity and diplomacy, during difficult and/or stressful situations to effectively collaborate and resolve issues.
-
Proven ability to work in a fast-paced environment, with demonstrated ability to manage multiple tasks, and customer requests, while meeting deadlines.

Pay Range - $22-$30/hour
While performing the duties of this position, exposure to changing weather conditions will occur.
