Position Summary
The Immigrant Social Care Navigator provides culturally responsive case management to immigrant and refugee clients within the Care Compass Network (CCN) framework. The primary role of the Case Worker is to provide direct, hands-on support to immigrants who have various immigration statuses such as Refugees, Humanitarian Parole, Assistance for Migrants Program (AMP) and other vulnerable populations funded by state programs, by helping them meet their immediate basic needs while guiding them toward long-term stability and self-sufficiency. This includes working closely with individuals and families to assess their circumstances, identify barriers, and connect them to essential services such as housing, food assistance, healthcare, employment resources, and public benefits with agency partners. The Case Worker serves as a trusted point of contact, helping clients navigate complex systems, complete applications, and access culturally and linguistically appropriate services.
In addition to addressing urgent needs, the Case Worker plays a critical role in advocating for clients, coordinating care across multiple service providers, and ensuring continuity of support through ongoing follow-up. The position emphasizes building strong, respectful relationships with clients, empowering them with knowledge and tools to independently access resources over time, and promoting overall well-being, integration, and stability within the community.
Key Responsibilities
1. Intake Assessment
· Conduct comprehensive screenings using the Care Compass assessment tools to identify housing instability, food insecurity, transportation, and utility needs.
· Assess specific immigrant-related needs, including language access, immigration needs.
· Verify eligibility for Medicaid Managed Care (MMC) programs and “Enhanced Population” services.
2. Resource Navigation & Referral Management
· Provide direct referrals to community partners for high priority needs such as housing, food, and health.
· Facilitate connections to English as a Second Language (ESL) programs, and employment training.
· Utilize social care platforms (e.g., Unite Us/Unite NY) to send, track, and close loops on electronic referrals.
3. Care Coordination & Advocacy
· Serve as a liaison between clients and healthcare providers to ensure appointments are kept and medical instructions are understood.
· Advocate for clients in complex settings (social services, schools, or housing offices) to ensure equitable access to benefits.
· Arrange for or provide medical and social service transportation coordination as needed.
4. Cultural & Linguistic Mediation
· Provide services in the client’s native language or coordinate with professional interpretation services to ensure clear communication.
· Assist clients in understanding the U.S. healthcare and social service landscape.
5. Documentation & Compliance
· Maintain accurate and timely electronic case files in accordance with Care Compass Network and Medicaid compliance standards.
· Track and report key performance metrics, such as referral outcomes and client progress toward stabilized social needs.
Qualifications & Skills
· Cultural Fluency: Deep understanding of the challenges facing immigrant, refugee, and asylee populations.
· Communication: Bilingual or multilingual proficiency is highly preferred.
· Technical Proficiency: Ability to navigate case management software and referral platforms (e.g., Unite Us).
· Empathy & Ethics: Strong commitment to client confidentiality and a trauma-informed approach to service delivery.
Preferred Experience
· Bachelor’s degree in human services preferred; equivalent work experience will be considered.
· Previous experience in social work, community health, or immigrant advocacy.
· Knowledge of local community-based organizations (CBOs) and health systems in the Binghamton/Southern Tier region.
Compensation
$22.00 hourly
