Position Overview:

As a Home Health Speech Language Pathologist, you will:

• Evaluate Patients: Assess patients' speech, language, cognitive-communication, and swallowing abilities to develop individualized treatment plans.
• Develop Treatment Plans: Create and implement therapy programs aimed at improving communication skills and swallowing function.
• Provide Therapeutic Interventions: Utilize evidence-based techniques to enhance patients' independence and quality of life.
• Educate Patients and Families: Offer guidance on communication strategies, swallowing techniques, and home exercises to ensure a safe and supportive environment.
• Collaborate with Healthcare Team: Work closely with physicians, nurses, and other therapists to coordinate comprehensive care.
• Document Progress: Maintain accurate records of patient evaluations, treatment plans, and outcomes in compliance with healthcare standards.

What We're Looking For:

• Licensed Professional: Active Speech Language Pathologist license in New York State.
• Experience: Experience in speech-language therapy; home health experience is preferred.
• Communication Skills: Excellent interpersonal skills to effectively interact with patients, families, and team members.
• Reliability: Strong organizational skills and the ability to work independently in a home setting.
