Seven Healthcare are seeking a qualified RN - Home Health professional to join their team in the Bronx Borough. This role requires providing care directly in patients' homes within the community setting. The position involves managing patients' healthcare needs in collaboration with other healthcare professionals.

Key Responsibilities of the RN - Home Health

• Assessing, planning, implementing, and evaluating patient care plans in consultation with healthcare professionals
• Evaluating medical information and providing direct and individualized bedside nursing care to pre-and post-surgery patients
• Preparing, administering, and recording prescribed medications
• Changing dressings, inserting catheters, and starting IVs
• Adjusting specialized medical equipment as needed and interpreting and recording measurements
• Monitoring patients' vitals and reporting adverse reactions to medications in a timely manner
• Collaborating on discharge planning with the healthcare team
• Answering questions and educating patients and their families on surgical procedures and post-surgery care
• Maintaining and updating patient files, medical charts, and health records
• Adhering to best nursing practices, standards, and protocols

Requirements of the RN - Home Health

• Minimum two (2) years Med/Surg experience in a large acute care hospital or Post Acute Facility
• Home Care Nursing experience
• OASIS proficiency
• New York State RN License & Primary Source Verification
• AHA BLS
• AHA ACLS (Queens only)
• Associates Degree in Nursing from an accredited educational institute
• Skills in physical assessment, medication administration, wound care & infection control, IV therapy, post-operative care, patient education, and telemetry monitoring
• EPIC EMR and knowledge of OASIS preferred
