You will coordinate with other members of the care team to ensure seamless care delivery and actively involve patients in their care planning. Primary Responsibilities: * Patient Care: Provide nursing care to patients via direct and/or telephone contact, following established standards and practices
* Team Coordination: Work with other care team members to ensure maximal coordination of efforts and active patient participation in planning and care
* Clinical Assistance: Assist physicians or other providers with clinical procedures and participate in patient teaching

Optum NY/NJ was formed in 2022 by bringing together Riverside Medical Group, CareMount Medical and ProHealth Care. The regional alignment combines resources and services across the care continuum - from preventative medicine to diagnostics to treatment and beyond across New York, New Jersey, and Southern Connecticut. Required Qualifications: * Graduate of an accredited school of Practical Nursing
* New York State (NYS) license with current NYS registration, in good standing or compact license Preferred Qualifications: * Current CPR/BLS certification
* 1+ years of experience as a licensed practical nurse or 2+ years as a Medical Assistant in a pertinent practice area Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.00 to $35.72 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
