Remote Clinical Pharmacist: Chronic Care & Team Collaboration
Humana Inc. is seeking a Clinical Pharmacist for our Conviva Senior Primary Care program in a remote role supporting multiple clinics. You will work with the care team to optimize medication therapy, provide patient education, and ensure high-quality outcomes.
The role emphasizes evidence-based pharmacotherapy, reconciliation, and collaboration with physicians and other healthcare professionals to improve patient safety and satisfaction. Must hold Florida licensure and have Pharm.D.
