Hospice Case Manager- Queens
- M-F 9:00am-5:00pm
- One weekend per month
- Backup On Call capacity 2-3 times per month
- Providing assessments to patients in the community, home, or facilities in designated territory
- Caseload of 14-18 patients; provide pain management, patient/family education
- RN, 2 years Acute any setting (Home/Hospital/Nursing Home)
- Drivers License and Car required
