DUTIES:
Partners with the DON to ensure that policies and procedures of the organization are up to date, in line with adjustments in regulations from NYSDOMH, Federal, and other regulatory bodies, and are adequately communicated and reinforced with the nurses as a part of a more significant communication and training strategy.
Collaborates with the DON to maintain policies and procedures for the effective implementation and tracking of monthly statistics for the nursing department.
Works closely with the QI team to support maintaining program quality standards, benchmarks, and performance measures regarding nurse activities and outputs.
Resolve conflicts and matters that could detriment the client and agency because of non-compliance of the nurse with established policies and procedures.
Lead the investigation and resolution of client incidents and complaints.
Collects data on common errors and omissions and present them at nursing meetings.
Assists actively with the remediation of any issues with the data management system; can learn the documentation system and seek assistance from customer service to resolve any issues with the technical aspects of nursing documentation and input.
Actively participates as a vital member of the agency Audit Team to ensure the correct submission of documents required during all in-person and remote and lead with remedying gaps and errors identified during the audit.
Execute visits for clients.
Provide training and coaching to the nurses supervised.
Provides feedback to the Director of Nursing on trends occurring in clinical documentation, including difficulties with completing documentation, timeliness of clinical documentation and nursing visits, and any issues relating to the compliance of nursing documentation and service delivery.
Participates in Continuous Quality Improvement activities under the supervision of the Director of Nursing and ensures that recommended improvements and post-audit corrective action plans are implemented.
Establishes and ensures implementation and completion of any/all corrective action plans for individual client cases as necessary.
Provide oversight over charts and preside over client case conferences as required.
Conducts internal chart audits as a monitoring tool to ensure the appropriate documentation by all staff per Agency procedures and all external mandates.
Participate in Nursing Quality Oversight Committee to assist in identifying issues, corrective actions, and implementing recommendations to improve the quality of the Nursing department function.
Other duties as assigned by the Director of Nursing.
Provide feedback to the Nursing Director on compliance/non-compliance trends.
Participates in the organization’s quarterly QI meetings, including QI Committee, Performance Improvement, and other meetings as mandated, to support addressing gaps in the nurse-client engagement program.
QUALIFICATION:
Minimum of a bachelor’s degree in human services, nursing, social work, management, or related fields; master’s degree preferred.
Required to be a licensed registered nurse.
A minimum of five (5) years of work experience.
3+ years of experience working in community and home health care.
2+ years of supervisory experience.
Good analytical, problem-solving, and decision-making skills.
Detail-oriented and precise with excellent organization skills.
Good computer skills and experience with Microsoft Office products, including Word, Outlook, Excel, and PowerPoint, is essential.
Ability to plan, multi-task, and manage time effectively.
Be a self-starter with the ability to learn quickly.
Ability to speak a relevant second language is desirable.
This job is satisfactorily performed when the incumbent meets the targets set for the following:
Review the weekly productivity report by nurses to identify gaps.
Manage weekly updates and feedback on positive and negative performance issues and facilitate/effect resolution as necessary.
Complete between 35-50 visits per month (all types of visits)/10-12 visits per week.
Evaluate nurses that have fewer than 20 visits per week to establish the reason for the shortfall.
