Essential Job Functions

  1. Provides transportation or accompanies the consumer to and from community activities.
  2. Follows the goals and/or activities specified in the individual's "Individualized Service Plan" and Community Habilitation Plan.
  3. Document/record events on the daily data sheet.
  4. Assists consumer in the development and support of social activities and relationships.
  5. Records observations and actions.
  6. Participates in staff development activities and programs.
  7. Works one-on-one in the consumers home and community to address a variety of life skills to include social, recreational and other activities of daily living.
  8. In cooperation with the individual and family, helps to identify skills and competencies to be developed and devises plans to achieve those skills and competencies.
  9. Assists consumers to identify social, recreation and other community resources the individual may wish to access and devise plans to achieve that access.
  10. Measures the likelihood of social isolation and develops plans to build social and communal relationships, which prevent or minimize that isolation.
  11. Negotiate schedules and daily activities.
  12. Regularly maintains and submits service notes with documentation of the hours worked and the service provided.
  13. Follows any agreed upon action plan for activities with the individual, including a behavior plan.
  14. Employs knowledge of emergency procedures as outlined by ICL.
  15. Enhances and promotes social interaction and community participation.
  16. Presents as a positive role model.
  17. Submits to and reviews reports and problems with the Residence Manager.
  18. Acquires and maintains current first aid, SCIP-R, and CPR certification.
  19. Other Duties as assigned

Essential Knowledge, Skills and Abilities

  • Committed to active promotion of ICL values and goals.
  • Resourceful within the community/able to network.
  • Knowledge of policies and procedures of OPWDD, as well as state and federal regulations as they apply to the care and treatment of the developmentally disabled.
  • Knowledge of active treatment programs as they relate to the developmentally disabled.
  • Currently SCIP-R certified or able to become certified within six months of hire.
  • Currently CPR and First Aid certified or able to become certified within six months.
  • Knowledge of basic computer skills Ability to operate a motor vehicle safely.
  • Ability to read and write at least at the secondary school level and follow written and oral instructions

Minimum Qualifications and Experience

Minimum: HS diploma or GED required. A valid New York State Driver's License and ability to drive and driving record acceptable for agency insurance coverage are required. CPR and First Aid certification or eligibility for certification. A minimum one year I/DD experience.

Preferred: Associates Degree
