Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient's recovery from illness, injury or surgery at home.

• Reporting to the Coordinator of Care Nurse, you will be responsible for seeing patients in their home for revisits based on a skilled need
• You will implement the plan of care and evaluate the treatment
• You will document the visit electronically and completing documentation within 24-48 hours

nursing, LPN, homecare, home care licensed practical nurse

• NY State LPN (Licensed Practical Nurse) License required
• Minimum of one year Med/surge experience in a hospital or Long-term Care facility required
• Car and driver license required
