Licensed Master Social Worker (LMSW)

Location: New York, NY 10016

Duration: 14 Weeks Contract

Shift: Day Shift

Hours: 9:00 AM – 5:00 PM | Monday–Friday

We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide psychosocial support, discharge planning, and care coordination services within a hospital setting. This role involves working closely with interdisciplinary teams to ensure patients receive comprehensive care and appropriate post-discharge resources.

Responsibilities:

• Conduct comprehensive psychosocial assessments for patients and families
• Provide supportive counseling and therapeutic interventions
• Identify emotional, social, environmental, and financial factors impacting patient care
• Assist patients and families in coping with illness, hospitalization, and recovery
• Develop and implement discharge plans in collaboration with the healthcare team
• Coordinate referrals to community agencies, rehabilitation services, behavioral health resources, and other support programs
• Facilitate safe and effective transitions of care
• Advocate for patient needs and appropriate support services
• Collaborate with physicians, nurses, case managers, and interdisciplinary teams
• Maintain accurate and timely documentation within the medical record
• Ensure compliance with hospital policies, regulatory standards, and social work ethics

Requirements:

• Master’s Degree in Social Work required
• Minimum 1 year LMSW experience required
• Hospital experience required
• Valid Licensed Master Social Worker (LMSW) license issued by NYSED required
• Strong psychosocial assessment and counseling skills required
• Experience with hospital discharge planning and care coordination required
• Excellent communication and interpersonal abilities required
• Ability to work effectively in a fast-paced healthcare environment required
