About the Practice
Burnt Hills Pediatrics and Internal Medicine, part of Community Care Physicians (CCP), is a well-established primary care practice providing compassionate, evidence-based care for patients of all ages. Our physicians are trained in Pediatrics or are dual board-certified in Internal Medicine and Pediatrics (Med-Peds), offering a unique ability to care for both children and adults within the same practice.
Serving the community since 1998, our team is dedicated to building long-term relationships with patients and families while delivering coordinated, high-quality care. We are recognized as a Patient-Centered Medical Home (NCQA), reflecting our commitment to accessible, team-based, and patient-focused care.

Position Summary
We are seeking a full-time Physician to join our collaborative primary care team. This role is ideal for a provider interested in caring for patients across the lifespan , with flexibility for those trained in Med-Peds, Internal Medicine, or Family Medicine.
The ideal candidate is committed to building long-term patient relationships, delivering high-quality outcomes, and working in a supportive, team-based environment.
Key Responsibilities
- Provide comprehensive primary care services to patients of all ages (pediatric through adult).
- Diagnose and manage acute and chronic medical conditions.
- Perform preventive care, wellness visits, and age-appropriate screenings.
- Deliver continuity of care across life stages, supporting both individual and family health needs.
- Collaborate with physicians, nurse practitioners, and physician assistants to ensure coordinated care.
- Utilize electronic health records, telehealth, and patient portal tools to enhance access and efficiency.
- Participate in quality improvement initiatives and practice development efforts.
- Educate patients and families on health promotion, disease prevention, and treatment plans.
