Registered Nurse – Home Health Job Details

• Location: Webster, NY (servicing Wayne County) – New York home health jobs
• Employment Type: Locum Tenens (Travel RN Contract)
• Start Date: August 18, 2025
• End Date: November 15, 2025
• Duration: 13 weeks
• Schedule: Monday – Friday, 8:00am – 4:30pm (8-hour day shifts), weekends as needed
• Hours per Week: 40
• Estimated Weekly Salary: $2250–$2310

RN Case Manager – Qualifications

• Active Registered Nurse (RN) license in New York State
• Minimum 1 year of clinical nursing experience in a hospital or acute care setting
• Certified home health care experience required
• OASIS assessment experience required
• Telemetry nursing experience required
• Experience with EPIC or Netsmart electronic health records preferred
• BLS certification (any provider accepted)
• ACLS certification (AHA only)
• Current NYS driver’s license
• Strong clinical assessment, interviewing, and communication skills
• Ability to work independently, exercise initiative, and adapt to change

Registered Nurse – Home Health Responsibilities

• Develop and revise patient treatment and discharge plans in collaboration with families, physicians, and interdisciplinary care teams
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, and discharges within agency timeframes
• Process physician orders, communicate patient needs and changes, and complete certification/recertification orders and discharge summaries
• Deliver specialized nursing services and supervise LPNs to ensure optimal patient health and safety
• Educate patients, families, and healthcare team members on disease processes, self-care, and preventive strategies
• Maintain accurate clinical records and ensure compliance with New York State and federal home health regulations
• Participate in patient care conferences, quality improvement initiatives, and peer reviews
• Promote a respectful, collaborative, and patient-centered environment for all staff and patients
• Ensure visit authorizations and documentation support medical necessity for home care services
• Demonstrate flexibility and willingness to visit patients throughout Wayne County as needed
