Job Title: Licensed Master Social Worker (LMSW)

Location: New York, NY

Duration: 13 Weeks

Schedule: Monday–Friday, 9:00 AM – 5:00 PM

Job Summary

We are seeking a Licensed Master Social Worker (LMSW) to provide comprehensive psychosocial support, therapeutic services, and discharge planning within a hospital setting. This role involves working closely with interdisciplinary care teams to ensure patients receive appropriate care during hospitalization and a smooth transition post-discharge.

Key Responsibilities

Patient Care & Support

• Conduct comprehensive psychosocial assessments for patients
• Provide supportive counseling and therapeutic interventions
• Address emotional, social, and environmental factors impacting patient care

Discharge Planning & Coordination

• Develop and implement effective discharge plans
• Coordinate referrals to community resources and post-acute services
• Ensure continuity of care following discharge

Care Team Collaboration

• Collaborate with physicians, nurses, and multidisciplinary teams
• Support inpatient care teams across medical/surgical and rehabilitation units
• Advocate for patient needs and appropriate care services

Required Qualifications

Education

• Master’s Degree in Social Work (MSW) – Required

Licensure

• Active Licensed Master Social Worker (LMSW) license issued by New York State – Required

Experience

• Minimum 1 year of LMSW experience required
• Prior hospital or acute care experience preferred

• Empathy, professionalism, and patient advocacy
