Clinical Supervisor - LCSW
Overview
We are seeking a compassionate and dedicated counselor to join our home healthcare team at CarePro of NY. The ideal candidate will assist OMH waiver participants who are experiencing significant problems in managing the emotional difficulties inherent in adjusting to a significant disability and/or living in the community. Experience in the waiver program is not required.
Responsibilities
- Develop and implement individualized care plans based on client assessments and needs.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide support and guidance to clients in navigating challenges related to their disabilities or personal circumstances.
- Conduct regular patient observations to assess physical and emotional well-being.
- Collaborate with social workers, nurses, and other healthcare professionals to coordinate comprehensive care for clients.
- Assist clients with daily living activities and promote independence through home community care initiatives.
- Maintain accurate documentation of patient interactions, progress notes, and care plans.
- Travel to patients for face-to-face sessions.
Requirements
The candidate must be a Licensed Clinical Social Worker, who can work in New York State. Additionally, the candidate must be able to travel to the patients.
