Case Management Coordinator (Remote / Miami-Dade Area)
Payrate: 34.14 an hour
Location: Work From Home (Candidates must reside within North Miami, Aventura, Hialeah, or Miramar , FL)
Travel Required: ≈ 50–75% local travel to member homes, assisted living facilities, and nursing homes within the region.
Work Schedule
- Monday–Friday, 8:00 am – 5:00 pm EST standard business hours.
- Remote training via Microsoft Teams.
We are seeking a self-motivated, energetic, detail-oriented, highly organized, tech-savvy Case Management Coordinator to join our Case Management team. This role involves coordinating and managing case activities telephonically and face-to-face to evaluate members’ medical needs and promote overall wellness.
Key Responsibilities
- Assess, plan, implement, and coordinate all case management activities with assigned members.
- Manage a caseload including supportive and medically complex members.
- Develop proactive care plans to address challenges and enhance member outcomes.
- Coordinate services including prior authorizations, PCP & specialist communication, medication review, and community resources.
- Facilitate compliance, satisfaction, and effective member advocacy.
- Identify quality/compliance opportunities and follow through on continuous improvement.
Required Qualifications
- Fluent in English & Spanish (speaking & writing) — REQUIRED.
- Valid High School Diploma or GED — REQUIRED.
- Bachelor’s degree (any field) — REQUIRED (social work, nursing, or related field preferred but not required).
- Case management experience — Required.
